Reporting to the Director of Care, this role offers the opportunity to make a meaningful impact by delivering high-quality, resident-centered care.The LPN is responsible for providing planned, patient-focused, goal-directed nursing care in accordance with provincial standards and best practices. Key Responsibilities
Provide direct resident care including medication and treatment administration Complete and maintain accurate clinical assessments, care plans, and documentation Monitor and evaluate care plans and update as required Communicate changes in resident condition to the care team Participate in care conferences and assist with scheduling as needed Support resident admissions, discharges, and transfers Collaborate with the interdisciplinary team to deliver person-centered care Demonstrate leadership, mentorship, and teamwork in daily practice
Qualifications and Requirements
Practical Nurse Diploma Current registration and good standing with the applicable provincial regulatory body Current CPR and First Aid certification Experience in long-term care or senior care preferred Strong computer skills including Microsoft Office Demonstrated leadership, coaching, and mentorship abilities Ability to complete a Vulnerable Sector Check (or willingness to obtain)
